Groin pain/swelling – Differential Diagnosis

Common causes

• Hernia
• Reactive Lymph nodes
• Sebaceous cyst
• Abscess
• Hidradenitis suppurativa
• Lipoma
• Femoral artery aneurysm

Must Not Miss

• Lymphadenopathy
  • Metastases
  • Lymphoma
  • Tuberculosis
  • HIV
  • Psoas Abscess

Easily missed

• Undescended testis
• Sarcoidosis
• Glandular fever

Red flags

• Hard fixed mass
• Persistent unexplained swellings >3 weeks
• Pelvic mass
• Systemic symptoms
  • (fever, night sweats, malaise, weight loss)

Investigations to consider

• FBC, CRP/ESR
• Glandular fever screen
• HIV screen
• Mantoux test
• CXR
• AXR/Hip X-ray
• USS
• FNAC/biopsy
• CT scan
